Additional visa pages
Additional visa pages will not be added to an expired, damaged or mutilated passport, or to passports that were issued with 1 year or less validity.
Please follow the instructions below and click on the links to download the relevant forms. Please disregard instructions and addresses on the online forms — these are not applicable when applying outside of the U.S.
Completing the application
Please type or print legibly in black ink when completing all sections of this application. Please print out and submit only one-sided pages of the application.
Please submit the following by mail to the Consulate General serving your area:
- Fully completed and signed application form (DS-4085). Please see notes on downloading forms.
- Current U.S. Passport
- Pre-paid self-addressed Express Post Platinum or Registered Post envelope for return of your passport.
- Applicants aged 15 years or below: Only one parent/guardian is required to sign the application form for a minor aged 15 years old or younger. The two parent consent rule for passport issuance does not apply and is not required for the addition of visa pages.
- Applicants aged 16 years or above: sign your own application
